To provide a service to adults who have learning disabilities mental health issues physical disabilities and/or older people in their own home and in the community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
The support will be provided by four staff teams located in Dundee.
