Number(s) and Age(s) of person(s) to whom service may be provided:
1. The service will be provided to older people, adults with a learning and/or physical disability, and adults with mental health support needs in their own homes and in the community.

2. The support will be provided by one staff team as agreed at registration. Where there are any subsequent changes the Care Inspectorate must be informed.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
