To provide a service to adults who may have mental health issues learning disabilities substance and alcohol misuse issues and physical disabilities living in their own homes and in the community.
The support will be provided by two staff teams located in Bathgate and Livingston. The Care Inspectorate should be informed of any subsequent changes.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
The Registered Manager is also the Registered Manager for (CS2019378297).
