To provide a support service care at home to adults who have social care needs living in their own homes and in the local community.
The service will be provided in the local authority areas of Stirling East Dunbartonshire South Lanarkshire and North Lanarkshire by one staff team.
The one staff team referred to in condition 2 will be based at the address agreed at Registration. The Care Inspectorate should be informed of any subsequent changes.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
