Any other conditions unique to the service:
1. To provide a support service to children and adults with support needs living in their home and in the community.
2. Where the support is provided from more than one address, the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided. 3. The service will be provided by 1 team located in Harris.
