To provide a service to adults and children with complex needs related to physical disabilities and/or learning disabilities living in their own homes and in their local community. The service will be provided to people living in the following health board areas; Highland Grampian Tayside Forth Valley Fife Lothians and Greater Glasgow and Clyde.
The service will be provided by 1 staff team covering; Highland Grampian Tayside Forth Valley Fife Lothians and Greater Glasgow and Clyde.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
