The service will be provided to adults and older people with physical and/or sensory requirements learning difficulties and/or mental health needs in their own home and in the community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
The service will be provided by three staff teams: one for Stirling and Clackmannanshire one for Falkirk and one for Glasgow North and South Lanarkshire.
The service will be provided within the Local Authority areas of Stirling Falkirk Clackmannanshire North and South Lanarkshire.
As agreed in the variation request dated 4 December 2025 the service will be provided within the Local Authority area of Glasgow until 30 November 2026. The Provider will submit an update to the Care Inspectorate by 30 September 2026 regarding plans for continued service provision within the Local Authority area of Glasgow.
