To provide a service to adults and older people living in their own homes and in the community.
The service will be provided by two staff teams.
Team one will support West Lothian and Falkirk areas.
Team two will support Stirling and Fife areas.
The support will be provided at the address agreed at registration. The Care Inspectorate is to be notified of any changes.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
