The service will be provided to adults in their own homes and in the community.
The service will be provided by one staff team located in Livingston. The service provider must keep the Care Inspectorate informed of any change to the address from which the service is provided.
The service will be provided in; Midlothian Borders West Lothian Dundee and Perth and Kinross local authority areas.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
The Registered Manager is also the Registered Manager for Curam Health Solutions Ltd (housing support service).
