The service will be provided to older people with assessed support needs in their home and in the community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided. The support will be provided by one team based in West Dunbartonshire supporting East and West Dunbartonshire and Glasgow as agreed in the notice granting the registration dated 31 July 2024.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
