The service will be provided to children and adults with complex/intensive care needs living in their own home and the community.
The service will be provided by two staff teams.
One team will be located in Lanarkshire supporting Lanarkshire and East Renfrewshire. The second team will support Stirling.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
