1. To provide a service to adults and older people with physical/sensory impairment and/or memory impairment/dementia, mental health needs and learning difficulties living in their own homes and in the wider community.

2. The support will be provided by two staff teams located at the addresses agreed at registration. The Care Inspectorate should be informed of any changes. 3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
4. The provider must update and re-brand all policies, procedures and care documentation by the 30 April 2019. The aims and objectives in respect of this care service must be reviewed and updated and a copy submitted to the Care Inspectorate by the 30 April 2019.
