Any other conditions unique to the service:
1. To provide a service to adults over 18 years and older people with mental health chronic conditions/disabilities substance abuse and/or learning difficulties living in their own homes and the wider community.
2. The support will be provided by two staff teams to South Lanarkshire North Lanarkshire and East Renfrewshire. 3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
