1. To provide a service to older adults, adults with learning/physical disabilities and adults with mental health support needs and children with support needs living in their own homes and in the community.

2. The support will be provided by two staff teams as agreed during this registration and where there are any subsequent changes the Care Inspectorate should be informed.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
