The service will be provided to adults and older people in North Lanarkshire and East Renfrewshire with assessed support needs in their own home and in the community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
The service will be provided by 3 staff teams located in Motherwell.
The number of care hours delivered by the service will not exceed 3522 per week. This will be reviewed by 23 July 2022.
