To provide a service to adults children and young people with disabilities which may include life limiting conditions and end of life care needs living in their own homes and in the community.
The support will be provided by 2 teams based in Edinburgh one for adults and one for children and young people.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
