Any other conditions unique to the service:
1. To provide a service to older people and people from the age of 16 years with physical disabilities learning disabilities and mental health issues living in their own homes. 
2. The support will be provided by 1 team as agreed in the notice granting registration dated 31 March 2016. 
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
