To provide a service to adults and older people who have physical disabilities Dementia or mental health support needs.
The support will be provided by two staff teams. The support will be provided in South Lanarkshire and North Lanarkshire.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
The registered manager is also the registered manager for CS2024000171 Housing Support Service.
