The service will be provided to adults and older people with physical or sensory needs learning disability and/or mental health conditions in their home and in the community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided. The service will be provided by 3 staff teams based South Lanarkshire.
The service will be provided in Glasgow East Renfrewshire Renfrewshire North and South Lanarkshire.
