The service will be provided to 14 people over the age of 18 who are experiencing homelessness and present with multiple and complex health and social care needs.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided. The service will be provided by one staff team operating from the care service address.
