The service will be provided to children young people and young adults who must have already been in receipt of the service with disabilities in their home in premises at 6 Eagle Street Glasgow and in the community.
When using the premises at 6 Eagle Street Glasgow the maximum number of service users will be 6 and will be 8 years of age and over. The premises may be used between the times of 9:00 am and 7:00 pm.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided. The service will be provided by four staff teams located in Glasgow.
