To provide support to older people and adults with autism learning disabilities physical health needs and mental health issues in Edinburgh and the Lothians Greater Glasgow Stirlingshire Perthshire and Lanarkshire living in their own homes and in the community.
The support will be provided by 1 staff team located at the service address as agreed at registration. The Care Inspectorate should be informed of any subsequent changes.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
