To provide a service adults with physical disabilities and older people in their own homes and in the community.
The service will be provided by one staff team located in Falkirk. The service provider must keep the Care Inspectorate informed of any change to the address from which the service is provided.
The service will be provided in; Falkirk Fife West Lothian Perth and Kinross and Edinburgh local authority areas.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
Support with medication will be limited to prompting and assistance only until such times as staff have completed training relevant to the level of support being provided.
