To provide a service to older people and people with mental health support needs living in their own homes and in the wider community in Stirling Falkirk and Clackmannanshire.
The service may also support a maximum of 5 identified individuals who have a learning disability or are Autistic.
The support will be provided by one staff team at the address agreed during registration and where there are any subsequent changes the Care Inspectorate should be informed.
The provider shall inform people using the service and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
